This is the reference page. If you want the story rather than the index, start with the main survey guide or the step-by-step walkthrough.

The basics

What is the GuestObsessed survey?

It is the official guest satisfaction survey run by Checkers Drive-In Restaurants, Inc., the company behind the Checkers and Rally’s burger chains. The address is printed on your receipt. You answer questions about a visit you have just made, and you receive a coupon for a free sandwich.

What is the GuestObsessed survey website?

guestobsessed.com — the plain address, nothing longer. It forwards you to the live form, which runs on Qualtrics. Old Index.aspx links published by other guides return 404.

Is guestobsessed.com legitimate?

Yes. It is the address Checkers prints on its own receipts, and it hands off to a checkers.qualtrics.com subdomain — Qualtrics being a major enterprise survey platform. Be more careful with the ring of look-alike sites around it, several of which send their ‘take the survey’ button to unrelated third-party domains.

Is the survey free?

Completely. No cost, no account, no app, no download. If something asks for a card number or a software install, you are not on the official survey — close it.

How long does it take?

Three to five minutes for a straightforward visit. Longer if you report a problem, because the form opens follow-up questions, and longer again if you write a proper comment — which we would encourage.

Who runs the survey?

Checkers Drive-In Restaurants, Inc., headquartered at 4300 West Cypress Street in Tampa, Florida. The software is Qualtrics, licensed by the company. This website is independent of all of them.

The survey code

Where is the 20-digit survey code on my receipt?

On current receipts, generally nowhere — and you do not need one. The code belonged to the previous survey platform. Read the full explanation.

Why does guestobsessed.com/Index.aspx not work?

Because that page no longer exists. Both legacy entry points — POS=Smartcode and POS=Piecemeal — return 404. Use the plain domain.

Can I take the survey without a code?

Yes. That is now the normal route. The survey identifies your visit from a receipt photo, or from the store number and the date and time.

What is the difference between a survey code and a validation code?

A survey code was an input — you typed it to start. A validation code is an output — the survey gives it to you at the end to redeem. The input code is retired; the reward at the end is not.

My receipt does show a long number. Should I use it?

There is nowhere on the current form to enter it. Receipts carry several long numbers — check numbers, order numbers, card references. Look for the store number and the date and time instead.

Taking the survey

What do I need to take the survey?

A Checkers or Rally’s receipt with a legible top block, an internet-connected phone or computer, and about four minutes. Nothing else.

Can I take the survey without a receipt?

No. There is no alternate entry route, no lookup by phone number, and no way to describe a visit from memory. The receipt is the only key.

What format does the date and time field want?

MM/DD/YYYY HH:MM on a 12-hour clock. An afternoon visit at 2:36 is 02:36 with PM, not 14:36. This causes more rejected entries than anything else.

How do I enter the store number?

Digits only. The field’s own example is 1234, so type 2072 rather than Store #2072. See the receipt guide.

Can I take it on my phone?

Yes, and it is the easier option — you photograph the receipt with the same device you are answering on. Turn off any VPN first.

How long after my visit can I take it?

Within the window on your receipt. Sources contradict each other on the exact number of days, so we will not print one as fact. Take it the same day and the question never arises — thermal receipts also fade, which is a separate deadline nobody mentions.

Is the survey available in Spanish?

We could not confirm a Spanish option on the live form, though older documentation and several third-party pages mention one. Expect English.

Do I have to answer every question?

Most rating questions are required to advance; the comment box is optional. Where a question does not apply, look for a ‘not applicable’ choice rather than guessing.

Will low scores cost me the coupon?

No. The reward is for completing the survey, not for approving of the restaurant. Answer honestly — inflated scores make a struggling location look healthy.

The reward

What do I get for completing the survey?

A coupon for a free sandwich. The exact item, and any required purchase, are set by the offer printed on your receipt — the survey says “see receipt” rather than naming one.

Do I have to buy something to use the coupon?

Almost certainly. Offers of this kind are structured as ‘free item with purchase’, historically a side and a drink. Plan for it.

Can I win $500 or a gift card?

We found no evidence any sweepstakes exists — no official rules, sponsor, prize value, entry period or no-purchase-necessary route, all of which US law would require. Disregard pages promising cash.

How often can I get a coupon?

The survey’s welcome screen states “One coupon sent per 7 days.” That limits coupons, not submissions — feedback given more often still reaches the company.

How long is the coupon valid?

Read your receipt. Thirty days is the figure most commonly quoted, and it is a reasonable planning assumption, but we could not verify it as a universal rule. Your receipt is the authority.

Which sandwich will I get?

Whatever your receipt’s offer names. Checkers and Rally’s run different promotions in different markets, which is precisely why the survey does not name one.

Is the coupon transferable?

Not formally — it is tied to your receipt. Whether a member of staff minds who is holding the paper is a different matter, but do not plan around it.

Redeeming it

Where do I write the validation code?

On the receipt, in pen, in the blank space beside the printed offer. That is where staff look. Screenshot it as well.

I closed the tab and lost my code. Can I get it back?

Generally no. It is displayed once, there is no resend, and restaurant staff cannot look it up. Check any email address you supplied, then treat it as a lesson.

Can I redeem at any Checkers or Rally’s?

At participating locations. Most restaurants are franchised and owners choose which promotions to honour, so the restaurant that served you is the safest bet.

What if the restaurant refuses my coupon?

Usually it is a non-participating franchise, an expiry, an unmet purchase requirement, or an attempt to combine it with another offer. Ask once for the manager, then contact the company — the person at the window has no authority over promotion policy.

Can I use it on a delivery app?

Effectively no. Third-party platforms have their own pricing and there is no way to hand over a paper receipt. Treat it as an in-person offer.

Can I combine it with another discount?

Assume not. Free-item offers are almost always marked non-combinable, and the usual friction point is trying to use one on top of an already-discounted value bundle.

I lost the receipt after taking the survey. Now what?

The receipt is the coupon. Without it there is nothing to redeem and no reissue process. Keep it until the food is in your hand.

When things go wrong

The survey page will not load. What do I do?

Turn off your VPN, pause your ad blocker, try mobile data instead of Wi-Fi, and check you are using the plain guestobsessed.com address rather than an old .aspx link. Those four cover most cases. More on the troubleshooting page.

My receipt photo will not upload.

Flatten the receipt, kill the flash, shoot straight down in daylight, fill the frame. After two failed attempts, switch to typing the store number and date — the fallback exists for exactly this.

My receipt has faded. Can I still take it?

If you can read the store number and the date and time by eye, yes — use manual entry. If not, that visit is spent. Thermal paper fades fastest in heat and sunlight.

The survey says I have already taken it.

Either that receipt has already been used, or you are inside the seven-day coupon window. Feedback in that window still counts; it just does not pay again.

Can I get a reprinted receipt?

Not for survey purposes. Restaurants do not reissue them and the survey cannot find your visit without one.

Who do I contact if the survey is broken?

Checkers Drive-In Restaurants. We are an independent guide with no access to the survey system. That said, if a screen looked different to what we document, tell us — it means our page needs re-checking.

The company

Are Checkers and Rally’s the same company?

Yes, since August 1999. Same menu, same operations, same survey. The two names survive because each is the familiar one in its own region.

Who owns Checkers and Rally’s?

Since a June 2023 recapitalisation, a group of long-time lenders: Arbour Lane Capital Management, Garnett Station Partners and Guggenheim Investments. Pages naming Oak Hill Capital Partners are out of date.

Who is the CEO?

Chris Tebben, president and CEO since September 2024.

How many locations are there?

More than 700 according to the company’s own 2026 materials, down from a peak above 800 earlier in the decade. Most are franchised.

When was Checkers founded?

1986, in Mobile, Alabama. Rally’s opened first, in Louisville, Kentucky in January 1985.

Why are the restaurants so small?

Because they were manufactured rather than built. Checkers produced prefabricated units of roughly 700 square feet through its own subsidiary, complete with equipment, installable in under three weeks and even relocatable. The brand page tells the whole story.

Is GuestObsessed part of Checkers Rewards?

No — two separate programmes. The survey pays a coupon for feedback; Checkers Rewards pays points for spending. Compared side by side on the rewards page.

Privacy and this site

What data does the survey collect?

Your store number and visit time, or a photo of the receipt, plus your answers. It may ask for an email address to deliver the coupon. It should never ask for a payment card, a password or a download.

Does this website collect my survey answers?

No. We have no survey form, no account system and no way to receive your receipt or your responses. Every survey button here opens guestobsessed.com. See our privacy policy.

Are you affiliated with Checkers or Rally’s?

No. We are an independent, ad-supported guide. We are not endorsed by, sponsored by or connected to Checkers Drive-In Restaurants, Rally’s, GuestObsessed.com or Qualtrics. See our disclaimer.

How do you make money?

Advertising displayed on these pages. Nobody pays for a mention, and no advertiser has any say in how we describe an offer. Our editorial policy explains the separation.

How do I report an error on this site?

Through the contact page. Corrections about the survey screens are particularly welcome, because a changed screen means our page is stale.

GuestObsessed Guide Editorial Team

We are an independent team that documents restaurant guest-satisfaction surveys. We are not Checkers, Rally’s, or the company that runs GuestObsessed.com. We take the surveys ourselves, screenshot what we see, date every claim, and correct pages when the survey changes. Read our editorial policy or tell us we got something wrong.
